Week 12 Documentation Log

Your Week 12 log is a procedure revision record.

Choose one failure from the supplied checklist or incident scenario and show how the documentation system changes because of it.

Failure mechanism

Name the actual mechanism:

  • missing checklist item;
  • weak wording;
  • stale evidence;
  • wrong checklist version;
  • bad sequence;
  • unclear failure response;
  • interruption/restart problem;
  • evidence lost after incident.

Incident evidence inventory

List at least five artifacts you would preserve before changing the simulated system:

  • payload evidence.

Mark which one would be easiest to accidentally overwrite.

Regression test

Write one future scenario that should fail the revised checklist if the old problem returns.

Then state the expected result.

Your answer should point to the current applicable rules, qualified operator/responsible organization, and school procedure—not memory.
