Incident Response

An incident is any event that interrupts safety, fairness, or operations. A response plan helps the team act calmly.

Response Steps

Use four steps: pause, document, route, recover. Pause the affected part of the event if needed. Document what happened. Route the issue to the right owner. Recover by restarting, rescheduling, removing harmful content, or escalating to an adult supervisor.

Worked Example

A player reports harassment in chat. The moderator pauses chat, screenshots the message if appropriate under school policy, removes the content, alerts the adult supervisor, and records the action taken.

Artifact: Incident Card

Create a response card for one incident: harassment, disputed result, equipment failure, missing player, or stream problem.

Common Failure

Do not solve conduct incidents privately in a side conversation with no documentation.

The response card should make the first three actions obvious.
